Case Management Services

APCare's Case Management Services provide a solid framework within which our staff can coordinate, provide and broker health, allied health, personal care, respite care, social support and other necessary services for clients with complex needs who require specialised care.

Our Case Management Services are predicated on a collaborative process of needs assessment, planning, facilitation and advocacy for options and services to be provided in the right balance so as to meet an individual's needs in the most cost-effective way.

Our staff are skilled at matching available resources and services to client needs and our person-centred mode of working ensures that we look to help each client maximise their level of independence.

Case Management Services

We can provide the following:

  • Arranging and attending Case Conferences
  • Drawing up a holistic care regime
  • Monitoring Health Plans
  • Booking and in-person facilitation/attendance at specialist appointments
  • Following up on interfaced care regimes from a range of health/allied health care providers
  • Brokering or booking specialist professional support (eg. Physiotherapy, Podiatry, Occupational Therapy, Dietician, Chiropractic, Speech Therapy etc.)
  • Providing Nursing Clinical Consultation and Assessment Services for people who already have personal support and other services in place but who need one-off or sporadic clinical consultation from qualified nursing staff
  • Arranging Medico-Legal Assessments
  • Liaising with government departments in regards to drawing up legal financial management arrangements
  • Coordinating and/or providing other care services not related to health needs but relating to care, social and lifestyle needs such as home maintenance
